This bulletin highlights recently produced documents that have appeared on websites. The aim is to trawl the web and produce a fortnightly review of what is found (coverage may not be comprehensive). This draft order sets out a proposal to abolish the Hearing Aid Council and transfer its register of hearing aid dispensers to the Health Professions Council. Closing date for comments is 7th July 2009.

Consultation on ordinary residence guidance, Directions and cross-border arrangements (21st April 2009)
This consultation document is seeking views on the revision of the Department of Health's guidance on the ordinary residence provisions in the National Assistance Act 1948 (current guidance is contained in LAC(93)7). It is also seeking views on draft Directions that set out local authority responsibilities when ordinary residence disputes arise and draft cross-border arrangements for dealing with ordinary residence disputes between English and Welsh authorities. Closing date for comments is 17th July 2009.

Diabetes guide for London (30th March 2009)
Diabetes is a major health issue for London, costing the NHS about £3million per day and affecting more than 350,000 people. Healthcare for London have announced plans to ensure that everyone with diabetes in London receives world class care. The guide sets out a plan to transfor diabetes care in the capital over the next three years, improving the standard of care provided, saving lives and reducing disabilities.

Minding the gap. The provision of psychological support and care for people with diabetes in the UK (April 2009)
According to this report, from Diabetes UK, around 85% of adults with diabetes in the UK lack access to specialist psychological support and care. It forund that only 25% of diabetes services can name and supply contact details for people who provide specialist psychological care.

Growing up with Diabetes: children and young people with diabetes in England (8th April 2009)
The Royal College of Paediatrics and Child Health (RCPCH) has found that almost 23,000 children and young people have diabetes in England - a figure higher than previous estimates. This snapshot survey gives accurate numbers of children and young people under the age of 18 with diabetes of any type in England.

This updated document, published by the National Treatment Agency for Substance Misuse (NTA)has revised references to the referral pathway process in light of recent changes announced by Jobcentre Plus. It also incorporates information with regards to travel expenses to a treatment assessment, references to support available, and a Jobcentre Plus process map to clarify the voluntary referral pathway to a treatment assessment.

Local intramuscular injection of botulinum toxin (BT) is an established treatment for spasticity, with strong evidence for its efficacy. The purpose of these guidelines, from the Royal College of Physicians, is to provide clinicians with the knowledge and tools to use BT effectively.

Latest figures, released by the Health Protection Agency, show that cases of the healthcare associated infection Clostridium difficile are continuing to fall.

Template Patient Leaflets on MRSA Screening (16th April 2009)
The NHS is now screening all relevant patients for MRSA before planned operations. This will help the NHS reduce the chances of patients getting an MRSA infection or passing MRSA onto another patient. These template patient leaflets have been developed to assist local areas in their patient communications around this topic.

Joint strategic needs assessment (20th April 2009)
This is a new publication from the IDeA's Healthy Communities Programme. It highlights the variety of approaches taken across the country and the huge amount that has been achieved. It also states that much more needs to be done to ensure all areas use the JSNA process effectively to improve the health and wellbeing of their population and reduce inequalities.

Intergenerational Demonstrator Programme: generations together (20th April 2009)
The Generations Together programme is seeking bids from all English local authorities with social services/children's service responsibilities to submit expressions of interest, in partnership with third sector organisations, for funding to develop demonstrator sites of intergenerational practice. It is a £5.5m programme which will run during 2009/10 and 2010/11.

Living Well with Dementia - a National Dementia Strategy: demonstrator site programme (7th April 2009)
The National Dementia Strategy requires an improved evidence base about the effectiveness of two service models proposed:
  • Objective 4 - enabling easy access to care, support and advice following diagnosis,
and
  • Objective 5 - development of structured peer support and learning, and we are inviting applications for demonstrator sites for these two areas.

In light of the new statutory duty to innovate, HSMC has been working with East Midlands Strategic Health Authority on a review of the evidence on planning implementing and embedding innovation within a health care setting. The review draws on evidence and examples of good practice from national and international literature and examines the range of innovation determinants highlighted in the literature. Finally, the review identifies a series of key message and recommendations for those seeking to innovate.

This briefing builds on the findings of the Audit Commission’s work on the Payment by Results data assurance framework and reviews the results of current data quality programmes. It also assesses how NHS trust and foundation trust boards assure themselves about the quality of their data. The research was carried out as part of the Commission’s national study of how boards get their assurance which they plan to publish in April 2009.

Lung cancer (non-small-cell, first line treatment) - pemetrexed: appraisal consultation (15th April 2009)
The Department of Health has asked the National Institute for Health and Clinical Excellence (NICE or the Institute) to conduct a single technology appraisal (STA) of pemetrexed for the first-line treatment of non-small-cell lung cancer (NSCLC) and provide guidance on its use to the NHS in England and Wales. The Appraisal Committee has had its first meeting to consider both the evidence submitted by the manufacturer and the views put forward by non-manufacturer consultees and commentators, and by the clinical specialist and patient expert representatives nominated for this appraisal by non-manufacturer consultees and commentators. The Committee has developed preliminary recommendations on the use of pemetrexed for the first-line treatment of advanced NSCLC. Closing date for comments is 6th May 2009.

Combined bony and soft tissue reconstruction for hip joint stabilisation in proximal focal femoral deficiciency (PFFD Interventional procedure guidance IPG297 (22nd April 2009)
Children are sometimes born with defects of the hip joint and upper thigh bone, called proximal femoral focal deficiency, often abbreviated to PFFD. Surgical treatment, aiming to produce as functional a leg as possible, may be appropriate. This may also involve a corrective operation on the hip joint (sometimes referred to as the ‘superhip' procedure). The initial operation may be combined with other surgery to lengthen the leg and treat other problems such as poor knee function at the same time or at a later date.

Endoscopic mastectomy and endoscopic wide local excision for breast cancer Interventional procedure guidance IPG296 (22nd April 2009)
Treatment for early breast cancer usually involves surgery to remove all or part of the breast. In this keyhole procedure, part or all of the breast tissue is removed using special instruments inserted through small skin incisions. The skin envelope of the breast and nipple are left intact, ready for an implant that can be inserted during the same operation.

Ex-vivo hepatic resection and reimplantation for liver cancer Interventional procedure guidance IPG298 (22nd April 2009)
The liver may be affected by cancer, either spreading from other parts of the body or developing from within the organ. In some patients, the tumour may develop in parts of the liver that are dangerous or impossible to treat with surgery, for example, when the tumour is very close to major veins that connect to the liver. This procedure involves removing the liver from the body, cutting away the diseased tissue, and reimplanting the remaining tumour-free liver into the patient.

Management of acute diarrhoea and vomiting due to gastroenteritis in children under 5 Clinical guideline CG84 (22nd April 2009)
This guideline applies to children younger than 5 years who present to a healthcare professional for advice in any setting. It covers diagnosis, assessment of dehydration, fluid management, nutritional management and the role of antibiotics and other therapies. It provides recommendations on the advice to be given to parents and carers, and also considers when care should be escalated - from home management through to hospital admission.
The guideline will assume that prescribers will use a drug's summary of product characteristics to inform their decisions for individual patients.

Glaucoma: diagnosis and management of chronic open angle glaucoma and ocular hypertension Clinical guideline CG85 (22nd April 2009)
The advice in the NICE guideline covers the diagnosis, treatment and care of:
  • adults (18 years and over) with a diagnosis of the condition known as 'chronic open angle glaucoma'
  • adults with a diagnosis of ocular hypertension (raised eye pressure)
  • adults who are at a high risk of developing glaucoma.
It does not specifically look at the treatment and care of:
  • people under 18 years
  • people with secondary glaucoma (glaucoma that is caused by other eye conditions, such as injury or inflammation)
  • people with, or at risk of, types of glaucoma known as 'angle closure glaucoma (primary or secondary)' or acute glaucoma
  • adults who have had glaucoma since birth or childhood.

Rivaroxaban for the prevention of venous thromboembolism Technology appraisal TA170 (22nd April 2009)
This guidance is about when rivaroxaban should be used to reduce the risk of thromboembolism in adults who have surgery to replace their hip or knee joints in the NHS in England and Wales. It explains guidance (advice) from NICE (the National Institute for Health and Clinical Excellence). It does not cover using rivaroxaban to reduce the risk of venous thromboembolism in adults who have had other types of surgery. It is written for people who are having or have had an operation to replace their hip or knee joints but it may also be useful for their families or carers or anyone with an interestin the topic.

Child wellbeing and child poverty. Where the UK stands in the European table (21st April 2009)
Child Poverty Action Group (CPAG) has published a briefing drawn from a new league table (produced by researchers from the University of York) of child wellbeing in European countries, in which the UK comes in 24th place out of 29 countries. The briefing also outlines the policies CPAG is calling for to end child poverty and improve child wellbeing in the UK.

This research explores the effectiveness of both existing and new questions on poverty, for use in the British Social Attitudes Survey. It considers among other things whether the public believe child poverty exists in Britatin today, what it is and who is responsible for tackling it.

National immunoglobulin database update: April 2009 (8th April 2009)
A key component of the DH-initiated National Demand Management Programme for Immunoglobulin is the National Immunoglobulin Database. This update document describes initial findings from the Database on immunoglobulin prescribing patterns, gives information on the forthcoming Model Commissioning Policy for immunoglobulin and offers a summary of the updated guidance on the use of immunoglobulin for measles post-exposure prophylaxis, issued by the Health Protection Agency.

In 2007, the HFMA and the Audit Commission published Primary Care Trust Accounts: A Guide for Non-executives. The guide was developed to help non-executive directors understand, draw conclusions and ask pertinent questions about a trust's accounts, and are a useful tool in getting to grips with the primary financial statements. An update to the guide has been produced in April 2009 to complement the original guides, describing the main changes to the accounts since publication and including additional questions for non-execs to assure themselves that the financial statements represent a true and fair view of the organisation's finances.

This guidance for funding excess treatment costs and subventions clarifies the directions set out in HSG(97)32. It relates to non-commercial research studies eligible for entry onto the National Institute for Health Research (NIHR) Clinical Research Network Portfolio database or studies funded through the NIHR Collaborations for Leadership in Applied Health Research and Care (CLAHRCs), Biomedical Research Centre and Biomedical Research Unit funding schemes.

Stroke-specific education framework (20th April 2009)
The overall purpose of the Stroke-Specific Education Framework (SSEF) is to create UK recognised quality assured and transferable standards for stroke training. It will also outline stroke-specific knowledge and skills which need to be added to ther generic skills that health, social, voluntary and independent care staff already possess. The Department of Health are now seeking comments on the Framework. Closing date for comments is 12th June 2009.

International Travel and Health 2009 Edition (17th April 2009)
International Travel and Health provides guidance to medical and non-medical people on potential health risks for travellers and precautions to take. The new 2009 edition includes a chapter on psychological health, detailed maps of major infectious diseases and updated vaccine recommendations and schedules.
