The National Patient Safety Agency (NPSA) today publishes Organisational Patient Safety Incident Reports for each NHS trust or local health board across England and Wales.

The Organisational Patient Safety Incident Reports summarise incidents reported by frontline NHS staff that have occurred between October 2008 and March 2009, and reported to the NPSA by 30 June 2009.

NICE was asked by the Department of Health (DH) to develop ‘Produce guidance on public health interventions to reduce accidental injuries to persons under the age of 15 on the road’. This evidence will be used by the Public Health Interventions Advisory Committee (PHIAC) to develop draft recommendations. Age-related macular degeneration is an eye disorder that usually starts later in life and causes problems with central vision. In this procedure, a combination of artificial lenses is implanted behind the pupil of one eye. The aim is that the implanted lenses help improve central vision while the other eye is used for peripheral vision.

Macular degeneration (age-related) - ranibizumab and pegaptanib Technology Appraisal TA155 (27th August 2008)
Ranibizumab is recommended as a possible treatment for people with wet AMD if all the following apply to their eye: The best possible visual acuity after correction with glasses or contact lenses is between 6/12 and 6/96; There is no permanent damage to the fovea (the part of the eye that helps people to see things in sharp detail); The area affected by AMD is no larger than 12 times the size of the area inside the eye where the optic nerve connects to the retina; and There are signs that the condition has been getting worse. Pegaptanib is not recommended for people with wet AMD. Healthcare professionals should not immediately stop prescribing pegaptanib for people who were already taking it when the guidance was issued. These people should be able to carry on taking pegaptanib until they and their healthcare professionals decide that it is the right time to stop treatment.

Pregnancy (rhesus negative women) - routine anti-D (review) (27th August 2008)
Routine antenatal anti-D prophylaxis is recommended as a treatment option for all pregnant women who are RhD negative and who are not known to be 'sensitised'. When a decision has been made to give routine antenatal anti-D prophylaxis, the treatment with the lowest cost should be used. The NICE guidance covers patients who are on a ventilator and have a tube placed into their mouth, nose or through the neck and down into their windpipe. Healthcare professionals should: make sure these patients are sitting up or leaning back supported for as much of the time as possible. This will not be appropriate for all patients (for example, those with injuries to the spine); and make sure an antiseptic is included as part of the mouth care routine for these patients.

Placement of pectus bar for pectus excavatum (Nuss procedure) (25th April 2009)
The Interventional Procedures Advisory Committee has considered this procedure and NICE has now issued a consultation document about its safety and efficacy. This has been issued for 4 weeks public consultation (until 5pm on 26 May 2009).

Pectus excavatum is an abnormality of the chest in which the breastbone sinks inward (sometimes called funnel chest). Problems associated with pectus excavatum are mainly cosmetic, although the condition can impair cardiac and respiratory function. Placement of a pectus bar for pectus excavatum (also known as the Nuss procedure) involves placing one or two steel (pectus) bars under the breastbone with the aim of raising it and correcting the abnormal shape. The bar, which is bent into a curve to fit the patient’s chest, is inserted through small openings in the chest. The bar (or bars) are usually removed within a few years of placement.

Balloon catheter dilation aims to keep sinus passages open by gently inflating a small balloon, which is introduced through the nose via a flexible tube. Interstitial photodynamic therapy for malignant parotid tumours (interventional procedures consultation) (2nd January 2008)
The parotid glands are located in front of the ears and help to produce saliva. Rarely, they may be affected by malignant tumours. In photodynamic therapy (usually abbreviated to PDT), a drug called a 'photosensitising agent' is injected into a vein. A few days later, special needles and fibres are inserted into the tumour and a special light is shone through them. The light causes the photosensitising agent to destroy the tumour cells. The National Institute for Health and Clinical Excellence is examining interstitial photodynamic therapy for malignant parotid tumours and will publish guidance on its safety and efficacy to the NHS in England, Wales, Scotland and Northern Ireland. Endoaortic balloon occlusion for cardiac surgery (interventional procedures consultation) (2nd January 2008)
During major heart surgery, the flow of blood through the heart needs to be stopped temporarily. In endoaortic balloon occlusion, a flexible tube (catheter) with a balloon attached to its tip is inserted into an artery in the groin (femoral artery) and threaded up to the heart. When the catheter is in the correct position in the heart, the balloon is filled with saline. As the balloon expands it blocks the aorta, which is the largest artery in the body. With the aorta blocked, the heart surgery can be performed. After heart surgery, the balloon and catheter are removed and the blood flow is restored. Allogeneic pancreatic islet cell transplantation for type 1 diabetes mellitus (interventional procedures consultation) (2nd January 2008)
Type 1 diabetes mellitus occurs when the body does not produce enough insulin (a substance that helps control sugar balance in the body). Although treatable with insulin injections, it is associated with an increased risk of other health problems, such as vascular disease. Allogeneic pancreatic islet cell transplantation involves the removal of islet cells, which are responsible for the production of insulin, from human donors. These cells are inserted into the patient's liver to restart insulin production within the body. Patients who have this procedure take medications so their body's immune system does not reject the cells. It is associated with some serious risks and is only considered in special circumstances. The National Institute for Health and Clinical Excellence is examining allogeneic pancreatic islet cell transplantation for type 1 diabetes mellitus and will publish guidance on its safety and efficacy to the NHS in England, Wales, Scotland and Northern Ireland. NICE

New database to inform prioritisation of European cancer research (20th October 2009)
A 'Database of Cancer Uncertainties' (DoCU) went live on the NICE website today. The database will collate details of the major gaps in evidence which are identified during guidance development. These evidence gaps can then be promoted as research priorities to prospective research funders.

The database has been developed as a pilot project by NICE, as part of the CoCanCPG (Coordination of Cancer Clinical Practice Guidelines in Europe). CoCanCPG is a consortium of 16 institutional partners from 11 countries.

NICE are consulting on the draft guidance ‘prevention of cardiovascular disease at the population level’. In particular, they are interested in the views of stakeholders on recommendations 3, 4 and 5 (on salt, saturated fats and trans fats). They are interested in views on:
  • Whether a figure for a lower target for salt intake could or should be identified at this stage
  • Whether an even lower saturated fat intake is achievable
  • The potential impact of a ban on industrial trans fats on cardiovascular disease in general and particularly the potential effects on existing inequalities in health.

Selective internal radiation therapy for colorectal metastases in the liver (interventional procedures consultation) (21st October 2009)
Colorectal liver metastasis is cancer that has spread to the liver from the bowel. Selective internal radiation therapy (known as SIRT) uses radiation put into the patient’s liver to kill the cancer cells. Tiny radioactive ‘beads’ are injected into the artery that supplies blood to the liver (the hepatic artery). The beads become trapped (embolise) in the tiny blood vessels surrounding the cancer, releasing radiation directly onto it. The treatment may be repeated.

Prosthetic intervertebral disc replacement in the cervical spine (interventional procedures consultation)(21st October 2009)
Bulging of a disc (called herniation) in the neck occurs when one or more of the spinal discs between the bones in the neck bursts and pushes against the spinal cord or nerve roots that run through the backbone. This herniation can cause pain in the neck, or pain, weakness and numbness in the arms and legs.

This procedure involves replacing damaged discs in the neck with artificial ones designed to act like natural neck discs.

Venous thromboembolism - reducing the risk: pre-publication check (22nd October 2009)
Following two consultation periods on the provisional recommendations, the National Clinical Guidelines Centre for Acute and Chronic Conditions (NCGC) and the Guideline Development Group (GDG) for Venous thromboembolism – reducing the risk, have considered and responded to stakeholder comments and amended the draft guideline.

The revised full guideline is now available on the web for a pre-publication check of factual errors. This is still draft guidance and has not been sent out to the NHS. The pre-publication check is not an additional consultation or an opportunity to reopen arguments and issues highlighted during consultation on the draft guidelines. The GDG will already have considered these issues in forming the recommendations contained in the draft for the pre-publication check.

Registered stakeholders for this guideline are invited to report any factual errors in the guideline by 11th November 2009.

Technology Appraisal TA182. Prasugrel for the treatment of acute coronary syndromes with percutaneous coronary intervention (28th October 2009)
Prasugrel in combination with aspirin is recommended as an option for preventing blockages in the arteries of people with acute coronary syndromes who are having a treatment called percutaneous coronary intervention only when:
  • immediate percutaneous coronary intervention is necessary to treat an ST-segment-elevation myocardial infarction (a type of heart attack) or
  • a blood clot has blocked a stent (a small mesh-like tube inserted into an artery to keep it open) during treatment with a drug called clopidogrel or
  • the patient has diabetes mellitus.

Technology Appraisal TA183. Topotecan for the treatment of recurrent carcinoma of the cervix (28th October 2009)
Topotecan in combination with cisplatin is recommended as a possible treatment for women with recurrent or stage IVB cervical cancer only if they have not received cisplatin before. Healthcare professionals should not stop prescribing topotecan for women who have received cisplatin before and who were already taking topotecan with cisplatin when the guidance was issued. These women should be able to carry on taking topotecan until they and their healthcare professionals decide that it is the right time to stop treatment.

Crohn's disease is a chronic inflammatory disease that affects the gastrointestinal tract. It is sometimes associated with other complications such as skin rashes and arthritis. In extracorporeal photopheresis blood is removed from the patient, then the white blood cells are separated from the whole blood, treated with ultraviolet light and re-infused into the patient.

The FAD has been sent to the formal consultees for this appraisal who have 15 working days to consider whether they wish to appeal against it. Subject to any appeal by consultees, the FAD may be used as the basis for the Institute's guidance on the use of the appraised technology in the NHS in England and Wales. The appeal period will close on 27th March 2009.

The Department of Health has asked the National Institute for Health and Clinical Excellence (NICE or the Institute) to conduct a single technology appraisal (STA) of sunitinib for the treatment of gastrointestinal stromal tumours and provide guidance on its use to the NHS in England and Wales. The Appraisal Committee has had its first meeting to consider both the evidence submitted by the manufacturer and the views put forward by non-manufacturer consultees and commentators, and by the clinical specialist and patient expert representatives nominated for this appraisal by non-manufacturer consultees and commentators. The Committee has developed preliminary recommendations on the use of sunitinib for the treatment of gastrointestinal stromal tumours. Closing date for comments is 26th March 2009.

NICE was asked by the Department of Health (DH) to develop public health intervention guidance for the NHS and local authorities on the prevention of skin cancer in the general public with specific reference to:
  • provision of information
  • physical changes to the environment
  • supply of sun protection resources
Registered stakeholders are now invited to comment on the evidence that has been assembled to inform the development of the first piece of guidance (Provision of information). Closing date for comments is 3rd April 2009.

NICE have been asked to develop a clinical practice guideline on Psychosis with substance abuse for use in the NHS in England, Wales and Northern Ireland. The draft scope defines what aspects of care the guideline will cover and to whom it will apply. Registered stakeholders for this guideline are invited to submit comments on the scope and may suggest clinical questions that could be answered in the guideline. Closing date for comments is 7th April 2009.
